Celebrating Life Community Health Center (CLCHC) has risen from humble beginnings to become a cornerstone of healthcare provision in Central and South Orange County. Founded in 2018, our journey has been marked by resilience, compassion, and an unwavering commitment to serving the community, particularly its most vulnerable members, including the homeless.
This year, the CalOptima Health Board of Directors, with the support of the city of Costa Mesa, has approved Celebrating Life Community Health Center to deliver essential medical services to unhoused populations in Costa Mesa as part of their Expanded Street Medicine Program. Street Medicine is a pioneering initiative that delivers primary healthcare, behavioral health services, and case management directly to unhoused individuals, bridging access gaps and providing compassionate care tailored to their unique needs. This partnership amplifies our commitment to serving and advocating for low-income and underserved individuals residing in Orange County.
The dedicated team at CLCHC is ready to provide a spectrum of essential services including medical, behavioral, and social services, directly where they are most needed. At the heart of the mission at CLCHC is a commitment to meeting individuals where they are and delivering high-quality care with dignity and compassion. By addressing not only individuals’ physical but also their social and emotional needs, CLCHC empowers people to reclaim their health and rebuild their lives.
By integrating healthcare with broader social support systems, CLCHC aims to create a pathway toward stability and improved health for our community’s most vulnerable members. Celebrating Life Community Health Center is excited to be at the forefront of this transformative initiative, embodying our core values of collaboration, community, and compassion.
